Abstract
Objective: To evaluate mismatch repair (MMR) protein status in endometrial carcinoma and its clinicopathological features in the Pakistani population.
Methodology: This cross-sectional study included 100 patients underwent hysterectomy or diagnostic endometrial curettage for endometrial carcinoma. Non-endometroid tumors were excluded. Clinicopathological data such as age, histological FIGO grade, and FIGO stage were recorded. MLH1, PMS2, MSH2, and MSH6 antibodies were used to determine the deficiency status on representative tissue blocks. SPSS 23 was used for statistical analysis.
Results: Mismatch repair protein deficiency was seen in 29% of the cases. The mean age of patients with MMR deficiency was 57.7±3.7 years. 78% of the MMR deficient tumors were FIGO grade 1 & 2 tumors with significant association (p 0.003). MMR protein deficiency was highest in stage I and II tumors (p<0.001).
Conclusion: Approximately 1/3rd of endometrial cancer cases in Pakistan have MMR deficiency, which is associated with old age (50-60 years) at diagnosis and lower FIGO grade and stage. These tumors are excellent candidates for anti-PD-1 immunotherapy and may show better outcomes.

Key words: MMR, endometrial carcinoma, IHC.
