The use of single dose Dexamethasone injection for the prevention of nausea and vomiting in post laparoscopic cholecystectomy patients.

Abstract
Introduction: Even today, nausea and vomiting are two of the most distressing adverse effects associated with post operative laparoscopic cholecystectomy. In the past few years important progress in the prevention of post operative-induced nausea and vomiting has been made mainly thanks to the introduction of preoperative Dexamethason, should be considered the treatment of choice. Recent reports have shown that Decadron has a significant antiemetic effect on post laparoscopic cholecystectomy-induced vomiting. Although the post operative peptic ulcer disease is a well known complication of steroids administrations. Decadron used as part of apost operative trend as antiemetic regimen for post operative nausea and vomiting is very effective, safe, and well tolerated. Almost all patients with preoperative decadron injection showed that they have identical antiemetic activity and tolerability, and it is preferred in patients who suffer from post operative vomiting. This study result is intended to alert others to the great effect of post operative nausea and vomiting prevention following short-term Decadron therapy.

OBJECTIVE: The objective of this study was to determine the efficacy of single dose (16 mg) dexamethasone in prevention of post lap. cholecystectomy-induced nausea and vomiting (12-24 h after surgery).
MATERIALS AND METHODS: well conducted comparative study has been done in Princess Haya Hospital of 60 patients took the decadron injection two hours prior surgery and another 60 patients as a randomized control group done prior to the study period ,this study has been done within a period of 8 months(1st January 2009-1st September 2009). All patients underwent regular assessments (every 2 h) post operatively for antiemetic control over the first 24 hours.
RESULTS: Sixty patients had single dose decadron injection 2 hours prior to operation. Nausea and vomiting were absent in 70% of them (42 patients), 13.3 %( 8 patients) had vomiting, while 16.6 %( 10 patients) had nausea in the first post operative period. Control group, only Nausea and vomiting were absent in 23.3% of patients (14), 36.6 %( 22 patients) had vomiting, while 40 %( 24 patients) had nausea in the first post operative period.76.6 %( 46 patients) had nausea and vomiting post operatively.
CONCLUSION: Dexamethasone injection decreased post operative nausea and vomiting after laparoscopic cholecystectomy.


















The proportions of patients who required rescue antiemetic (metoclopramide) with ringer lactate infusion drip were 7 patients (11.6%) in the dexamethasone group, while 20 patients (30%) needed rescue antiemetic and intravenous infusion in the control group.
