Validation of new clinical bedside test 'babur sign'for identification of wrist joint synovitis in patients with inflammatory arthritis

Abstract
Objective: To demonstrate a new clinical test (Babur sign) in patients presenting with inflammatory arthritis involving wrist joint keeping musculoskeletal ultrasound as gold standard.
Methodology: This study was conducted at Rheumatology department, Fauji Foundation Hospital, Rawalpindi from June to December 2021 and included 196 patients. All patients had musculoskeletal ultrasound. Babur sign was performed when wrist hyperflexion performed by the clinician results in immediate shoulder abduction along with elevation of the elbow in patients with active synovitis. Data were analyzed using SPSS 22.
Results: Out of 196 patients, 98% were female. Mean age was 33±6.7 years. 122 (61.6%) patients had Rheumatoid arthritis, 28 (14.1%) had psoriatic arthritis and 29 (14.6%) had Juvenile idiopathic arthritis. Systemic lupus erythematosus was found in 18 (9.09%) patients and Stills disease was diagnosed in only one patient. Babur sign was performed synovitis was seen in 157(79.2%) patients.
Conclusion: New clinical sign (Babur sign) is a reliable tool to pick up synovitis in patients with inflammatory arthritis.
