Efficacy of triamcinolone acetonide injections in the management of oral submucous fibrosis at a tertiary care hospital, Karachi, Pakistan

Abstract
Objective: To evaluate the efficacy of intralesional triamcinolone acetonide injections in the management of oral submucous fibrosis (OSF) at a tertiary care hospital in Karachi.
Methodology: This quasi-experimental study was conducted in the Department of Oral and Maxillofacial Surgery, JPMC, Karachi, from March to September 2025. Eighty-eight clinically diagnosed OSF patients were recruited by convenience sampling. Each received weekly submucosal 1.5ml dose of triamcinolone acetonide with lignocaine for four consecutive weeks. Mouth opening, burning sensation, mucosal stiffness, and blanching were recorded after each dose. Data were analyzed using SPSS 20. For assessment of association among the clinical outcomes, a chi-square test was used, and p<0.05 was set as statistically significant.
Results: Among 88 patients (mean age 39.8±11.6 years; 72.7% males), areca-nut–related habits were common (paan 56.8%, gutka 52.3%). Significant dose-related improvements were observed in all clinical parameters (p<0.001). By the fourth weekly injection, a definite increase in mouth opening and complete reduction of the burning sensation were achieved in 100% of patients, blanching improved in 93.2%, and stiffness in 72.7%.
Conclusion: Weekly intralesional triamcinolone acetonide injections offer a simple, cost-effective, and effective therapy for reducing symptoms and improving oral function in patients with OSF.
