Comparison of the postoperative pain in Lichtenstein inguinal hernia repair under self-applied local anesthesia versus spinal anesthesia

Abstract
Objective: To compare the mean postoperative pain in Lichtenstein inguinal hernia repair under self-applied local anesthesia versus spinal anesthesia.
Methodology: This Randomized controlled trial was done at the Department of General Surgery, DHQ Teaching Hospital, Gujranwala from 1st September 2019 to 31st March 2020. A cohort of 70 male patients with inguinal hernia were randomized into two groups utilizing a lottery-based method. Group A, underwent Lichtenstein inguinal hernia repair under the administration of self-applied local anesthesia, whereas Group B underwent surgery with the application of spinal anesthesia. After 24 hours observation period, patients were assessed of their pain level, utilizing the visual analogue scale (VAS). Data were analyzed with SPSS 25.
Results: Mean age of patients was 52.29±14.49 years. Mean postoperative pain in group-A was 1.31±0.68 and in group-B 2.83±1.29 (p<0.0001). The data stratification for age, BMI, duration of disease and side of hernia showed that statistically significant difference was evident between all subgroups of confounding factors.
Conclusion: Mean postoperative pain was less in Lichtenstein inguinal hernia repair under self-applied local anesthesia versus spinal anesthesia. Hence, self-application of local anesthesia may help in early pain reduction and can reduce analgesia requirement.
