Clinicopathologic spectrum of colorectal polyps highlighting alarming frequency of pre-neoplastic adenomatous polyps with high grade dysplasia

Abstract
Objective: To assess the prevalence of colorectal polyps along with clinicopathological characteristics in a tertiary care hospital.
Methodology: This retrospective study was performed at Histopathology department of Dr. Ziauddin Medical University and Hospital. A total of 356 biopsies from January 2018 to December 2022 (5 years periods) were included along with clinical data of patients. Biopsy tissue samples were stained with Hematoxylin and Eosin stain (H&E) and examined under light microscope to characterize the polyps on histomorphology basis.
Results: Amongst 356 biopsy tissue samples of colorectal polyps, 63.2% were males and 36.8% females. Patients more than 60 years old were 40.16%. Most common clinical symptom was rectal bleeding (46.34%) and most common site was rectum. Adenomatous polyps were 40.44%, hyperplastic 26.12%, juvenile polyps 25.28%, Inflammatory 7.86% and adenocarcinoma 0.28%. Adenomatous polyps with severe dysplasia were 48.6%;46.52% showed mild dysplasia and 4.86% showed moderate dysplasia.
Conclusion: Our study highlighted the prevalent clinicopathological features of colorectal polyp in our set of population along with alarming frequency of pre-neoplastic adenomatous polyps. Importance of close surveillance and screening for malignant transformation and prompt surgical removal of adenomatous polyps with high malignant potential is conclusively suggested for timely prevention and treatment.
