Clinical insights on post-chikungunya viral arthritis: A survey of Rheumatologists and Medicine Consultants

Abstract
Objective: To evaluate current diagnostic and therapeutic practices among rheumatologists and medicine consultants in Pakistan regarding post-Chikungunya viral arthritis.
Methodology: This cross-sectional online survey was conducted between January 31 and March 30, 2025, using a 13-item questionnaire. Clinicians across Pakistan were invited; 123 responded (19.1% response rate). Data were analyzed using SPSS 24, with significance set at p≤0.05.
Results: Out of 123 respondents, 51.2% were rheumatologists and 48.8% medicine consultants; 91.9% practiced in urban settings. A high proportion (88.6%) reported clinical exposure to Post Viral Chikungunya Arthritis. Rheumatologists were significantly more likely to suspect post-viral chikungunya arthritis in patients with persistent arthritis and recent febrile illness (p<0.001) and employed more comprehensive diagnostic strategies. Treatment approaches differed significantly: rheumatologists favored corticosteroids and DMARDs, while medicine consultants preferred NSAIDs and analgesics (p<0.001). For refractory cases, rheumatologists commonly used DMARDs, whereas medicine consultants increased corticosteroids or referred patients (p<0.001). Awareness of guidelines was limited, yet 95.9% supported the need for standardized protocols and further research.
Conclusion: Significant variability exists in the diagnosis and management of Chikungunya arthritis across specialties in Pakistan. The findings underscore the urgent need for evidence-based guidelines to ensure consistent and effective patient care.
