Functional outcome of PHILOS plate fixation in proximal humeral fractures

Abstract
Objective: To determine the frequency of functional outcome follows proximal humeral interlocking system (PHILOS plate) fixation in proximal humeral fractures at department of orthopedics and trauma, Khyber Teaching Hospital, Peshawar.
Methodology: This cross sectional study was carried out in six months at Khyber Teaching Hospital Peshawar. Surgical procedure was carried out using a standard deltopectoral approach on 153 patients. Postoperatively, the shoulder was immobilized in an arm sling for 2weeks. Active assisted motion was limited to 40 degrees up to six weeks and patients followed. All patients received a similar physical therapy program. Functional outcome was assessed using the Constant-Murley score at follow-up of 6 months. The Constant-Murley score was graded as poor (0-55 points), moderate (56-70), good (71-85), or excellent (86-100). Data was collected and analyzed by SPSS version 20.
Results:. Out of 153 patients, 80(52.28%) were males whereas 73(47.71%) females. Mean for age of patients was 45±13.2 years. For functional outcomes, 19(12.41%) patients had poor results, 30(19.60%) had fair results, 37(24.18%) had good results and 67(43.79%) had excellent results.
Conclusion: PHILOS plate fixation in proximal humeral fractures for 2-part and 3-part fractures had remarkable functional outcomes. Its use in 4- part fractures is associated with less desirable outcomes in comparison to 2 and 3 part fractures.
