Functional outcome in closed comminuted subtrochantaric fracture of femur in adults using dynamic condylar screw

Abstract
Objective: To evaluate the functional outcome of Dynamic Condylar Screw (DCS) fixation for closed comminuted subtrochantaric fracture of femur in adults.
Methodology: This cross sectional study was performed in the Department of Orthopedic and Trauma, Lady Reading Hospital Peshawar from December 2014 to June 2016. It included 197 patients with closed comminuted subtrochantaric fracture of femur. The fracture was reduced on a standard orthopedic traction table under C-arm guidance and was followed by open reduction and stabilization with DCS and side plate. Functional outcomes were assessed using the Harris hip score at 6th and 12th month after surgery.
Results: Mean age was 52±8.45 years. Out of 197 patients, 72% were male while 28% female. Seventy two percent patients had acceptable outcome while 28% patients had not acceptable outcome. 43(22%) patients had excellent functional outcome with Harris Hip Score 90-100, 99(50%) had good functional outcome with Harris Hip Score 80-89, 39(20%) had fair functional outcome with Harris hip score 70-79 while 16(8%) had poor functional outcome with Harris Hip Score <70.
Conclusion: Dynamic Condylar Screw is an easy, economical, less technically demanding and rewarding method of treatment for comminuted sub-trochantaric fracture of femur in adults in our part of the world.
