Role of fibrinogen in predicting severity of diabetic foot ulcer

Abstract
Objective: To find whether the fibrinogen levels can be used as valuable biomarker for severity and progression of Diabetic foot ulcer (DFU).
Methodology: In this retrospective study, 150 patients with DFU were selected at Cantonment General Hospital, Rawalpindi, Pakistan from January 2016 to January 2017. The severity was assessed by Wegener criteria. Age, gender, duration of DM, fasting plasma glucose (FPG) levels, hemoglobin, WBC counts, fibrinogen levels and status of amputation were considered. Data were analyzed by SPSS version 24.
Results: Patients with DFU were divided into two groups based on Wegener criteria. The mean age was 57.85±15.84 years. Male to female ratio was 89 (59.3%):61 (40.7%). The mean fibrinogen levels were higher in the patients with DFU grade 3-5 then in grade 1-2 (5.42±0.87 versus 2.92±0.57 g/L, p <0.0001). There was no difference between groups in terms of age, gender, duration of DM, FPG levels, hemoglobin and WBC counts (p values 0.592, 0.058, 0.534, 0.237, 0.731 and 0.932, respectively). The fibrinogen levels were higher in patients undergoing minor or major amputations (p<0.0001).
Conclusion: Fibrinogen levels are good predictor of severity and a good tool for progression of disease in patients of DFU.
