Club foot management by accelerated Ponseti technique

Abstract
Objectives: To evaluate the outcome of accelerated Ponseti technique for the treatment of clubfoot.
Methodology: This prospective experimental study was conducted at orthopedic and spine surgery unit Hayatabad Medical Complex, Peshawar and a private clinic from December 2013 to July 2014. A total of 28 patients (40 feet) with congenital telipes equinovarus (CTEV) were included in the study. Patients with other congenital anomalies and previous surgical or non surgical treatment for club foot were excluded from the study. PatientÂ’s feet were scored according to Pirani scoring system. Accelerated Ponseti technique (2 casts per week) was applied on all patients. On each cast removal, improvement in Pirani score was noted. Primary outcome measure was deformity correction.
Results. Out of 28 Patients, males were 16(57%) and females were 12(43%). 12(43%) had bilateral CTEV and 16(57%) were unilateral. In unilateral feet, 11(40%) were right sided and 5(17%) were left sided. Median Pirani Score was 5.2. Percutaneous tenotomies of tendo achillis were done in 32(80%). 34(85%) feet were fully corrected (Pirani score = or <1)
Conclusion: Accelerated Ponseti technique for club foot management is equally effective in relatively shorter period of time.
